Tadalafil (Cialis), marketed as Cialis (Tadalafil), is a prescription medication primarily used to treat erectile dysfunction (ED) in men. Recent studies have shown that tadalafil may have a negative effect on cardiovascular health, possibly by affecting phosphodiesterase type 5 (PDE5), which is a key enzyme that is responsible for the breakdown of cyclic guanosine monophosphate (cGMP) in the corpus cavernosum. Additionally, tadalafil is also used in erectile dysfunction patients who have experienced a significant decrease in blood pressure, which may be due to the reduction in blood pressure being caused by the inhibition of PDE5. We aimed to assess the safety and pharmacokinetics of tadalafil in patients who have been prescribed this medication. A total of 4,065 patients were evaluated, of whom 374 (30.6%) had a history of tadalafil-associated ED. All patients received a single dose of 20 mg, 40 mg, or 80 mg of tadalafil once daily. Mean Tadalafil concentrations were compared between the 20 mg, 40 mg, and 80 mg doses. The incidence of adverse events was significantly higher in the 80 mg tadalafil group, and the adverse events were significantly lower in the 20 mg and 40 mg groups (P <0.01). In the 80 mg group, the incidence of the gastrointestinal side effects (including flatulence, constipation, and dyspepsia) was significantly lower, but the incidence of the cardiovascular side effects was not significantly different from the 20 mg group (P <0.05). Although this study is limited by small sample size and retrospective observational design, this finding may help establish a safe and long-acting regimen of tadalafil in patients who are experiencing erectile dysfunction. Therefore, future studies should address the safety and pharmacokinetics of tadalafil in patients who have been prescribed this medication.
